We are continuing with our COVID-19 office protocols. For new patients, you will receive 3 emails. One goes over all of the office protocols, one explains how to complete the new patient paper work, and one is a pre-appointment screening questionnaire. The pre-appointment screening questionnaire should be completed on the day of your appointment prior to your appointment time by replying to the email and answering the questions. All existing patients will continue to receive the pre-appointment screening questionnaire via email prior to their appointment and should complete it accordingly.
